I/We hereby agree to contract Mi Plumber, to maintain and service the onsite sewerage waste treatment unit on the above-mentioned property.

Terms #2(Required)

I/We understand that it is a requirement of State and/or Local Government that our onsite sewerage waste treatment unit be maintained on a quarterly basis and that a Maintenance Agreement be renewed annually, and that this agreement is for a period of 12 months.

Terms #3(Required)

I/We also agree to allow the representative from Mi Plumber safe and suitable access to the above Treatment System at all reasonable times as they may deem necessary to carry out such service and maintenance.

Terms #4(Required)

I/We agree to pay all relevant fees and charges for each service as it falls, and, any extra service or maintenance charges that may arise from general equipment failure, or the Treatment System not being operated in accordance with the Owner’s Manual.

Terms #5(Required)

Mi Plumber hereby undertakes to service, maintain, and perform the necessary tests for the above Treatment System for a period of twelve months as outlined above and to send all relevant information to the Local Authority.
